INVESTOR REGISTRATION Why come and General Information For Investors
28th Angel Forum - Tuesday, October 19, 2010 - 7:30 am to 6 pm, main floor of Simon Fraser University downtown, 515 West Hastings St., Vancouver, Map of Streets / Parking and nearest hotel
Annual Members are investors who register in the "membership" category:
- register just once at a preferential rate for all Angel Forums until August 31, 2011.
- use of our co-investment / syndication software for your Angel Forum or other deals.
- can bring an investor friend new to the Angel Forum at no cost.
- are entitled to the member rate (usually 20% less) for workshops such as
Angel Investing Overview and Advanced Valuation / Post Investment Relationship.
- receive priority invitations to other investment opportunities as well as other networking events.
Attend this Forum only - for investors who register for one Angel Forum at a time, but are not members.
Investor Friends / Spouses are invited at a 50% fee if you register them on your registration at the same time.

- By registering, you are attending solely to consider investing $100,000+ and NOT to offer other services.
- Be aware of BC Securities Commission requirements for Accredited Investors and the $150,000 exemption
- You agree that private equity investing has a high risk of capital loss and you will seek professional advice.
- Your last name and contact information will not be released to anyone without your consent.
- Only your first name, company name and industry interest will be released.
- Everyone must pre-register as there is no registration on Forum day.
- Your registration/payment will be receipted via e-mail from system-mail@confmanager.com.
- You will receive by e-mail further details approx. one week before the Forum date.
- No refund within 60 days of the event date. You can send a substitute if you advise us and they qualify.
- Until your Investor Registration fee is paid, you can not access the Company Profiles and related pages.
- Company Profiles will not be published until approximately one week before the Angel Forum date.
- A User ID / Password emailed to you, allows viewing of Companies Profiles and collaborate with investors.

Immediately after signing up and staying in the website, use "Investor Registration" link to register for the Angel Forum.
If you leave the website after sign-up, you can re-access using the User Id and Password emailed to you almost immediately after sign-up. Use the "Investor Registration" page to register for the Angel Forum.
Forgot your User ID or Password? It will be automatically emailed to you after you have inserted your User ID (usually your first name and last name combined with no space between). Passwords are randomly generated, but it can be amended by the user after logging in. Forgot both your User ID and Password? E-mail us for both to be sent to the email address you signed up with.
Please do not sign-up twice as you will receive different userids and different passwords each time and you will encounter difficulties in accessing specific pages linked to you specifically. Problems ? please email us.

Fine Print:
User ID and Password is for access to "private pages" allowing Investors to register for the Angel Forum and Registered Investors to a) view online the selected Company Profiles (accepted to present one week before the Angel Forum date) and b) to discuss with other Registered Investors, an individual company that may require more than one investor to fund. By signing-up, you allow us to use your email address to notify you of future Angel Forum and related information and to immediately provide you with a User ID (your first name and last name combined with no space between) and Password to allow Investors to access private information pages.
H1N1 (Swine) Flu Precautions: On the workshop day, please do not attend if you feel sick and we will mail the presentation book to you. We will have alcohol based hand washing gel dispensers and ask that people wash their hands at each break period and to refrain from shaking hands etc.
Security of Personal and Payment Information: Your registration and payment will be processed through Global Payments (one of the largest transaction processing service providers in the world, listed on the NYSE), Beanstream (services about 20% of Canada's ecommerce marketplace) and Conference Manager (an application services provider specializing in conferences).
Online Payment of Fees using Credit Cards: Only Visa and MasterCard credit cards are accepted by our secure online registration system. "Beanstream" will be the merchant name on your credit card statement. Your receipt will be the email confirmation sent to you - GST# 882423387 is for "Angel Forum" the organizers.
Updating your Payment? Need to make a registration payment? Click here to log-in and make a new payment.
Registration Confirmation: Your registration and payment will be acknowledged via e-mail with confirmation of your requirements according to your registration form. This will be your receipt. Your registration will not be processed or confirmed if payment does not accompany your form. You will receive by e-mail further details approx. 1 week before the Forum date. At our check-in desk, you will receive your package containing all of your material including the book of Company Profiles.
Cancellation Policy: Only email cancellations to us marked "Cancellation" will be accepted until 60 days before the event date. A full refund will be made back to your credit card. No refund is possible after that date, but your registration may be transferred to another delegate.
Insurance: Registration fees do not include insurance of any kind. It is strongly recommended that at the time you register for the conference and book any travel, you take out an insurance policy of your choice. The policy should include loss of Forum fees/deposit through cancellation of your participation in the Forum, or through cancellation of the Forum, loss of air fares through cancellation for any reason including airline delay or related services strikes, Force Majeure or any other reason, medical expenses, loss or damage to personal property, additional expenses and repatriation should travel arrangements have to be altered. We cannot take any responsibility for any participant failing to arrange their own insurance.
